Mr. Kenneth Frazier and Miss Rose Miller, affidavit having been filed in this office, as provided by Public Act No. 128, Laws of 1887, as amended, by which it appears that said Kenneth Frazier is 22 years of age, color is W, residence is Carney, Michigan, and birthplace was Pine City, Minnesota, occupation is Farmer, father’s name Edward Frazier, and mother’s maiden name was Bertha, has been previously married – no times; and that said Rose Miller is 18 years of age, color is W, residence is Carney, Michigan and birthplace is St. Francis, Wis., occupation is Home girl, father’s name Frank Miller, and mother maiden name was Mary Blue, and who has been previously married – no times.

Frazier; Relationship of That Person: Wife; Address of That Person: Daggett, Menominee, Mich.; Employer’s Name: Alex Oadroski; Place of Employment: Daggett, Menomiee County, Mich.; Race: White; Height: 5′ 6″; Weight: 176; Eyes: Blue; Hair: Brown; Complexion: Light; Other obvious physical characteristics that will aid in identification: Scar on right side of eye; Signed: October 16, 1940”

According to the Stevens Point Journal (Stevens Point WI) Saturday, December 30, 1972, page 2:

“Man Dies of Heart
Attack In Auto

   Kenneth S. Frazier, 55, of Woodruff, was pronounced dead on arrival at St. Michael’s Hospital in Stevens Point Friday after he suffered a heart attack while driving his car.
   Portage County Sheriff’s Department deputies said Frazier was driving west on Highway 10, 2 1/2 miles east of Stevens Point, about 11:15 a.m. when he blacked out.  The car ran off the roadway into the north ditch.
   Following an autopsy Portage County Coroner Emil E. Przekurat confirmed that Frazier died of a heart attack.  There was no damage to the car.

“Kenneth S. Frazier, head, age 34, married, born in Minnesota, Occupation: Truck driver – Forest Products; Rose Frazier, wife, age 28, married, born in Wisconsin; Kenneth A. Frazier Jr., son, age 8, never married, born in Wisconsin”
