“ADDITIONAL CHARGES PENDING IN FATAL PARK CITY CRASH
PARK CITY – Additional charges are pending against a Woodstock-area man involved in a fatal crash Saturday, Police Chief Michael Luff said Tuesday.
The man, initially identified as Ustocio Porcayo, drove his pickup truck in front of a motorcycle on Belvidere Road, causing a crash that injured two people, one of whom died Monday.
Donna Marie Lubeno, 44, of Volo died Monday afternoon at Provena St. Therese Medical Center in Waukegan.
The collision occurred 11:30 a.m. Saturday at Staben Street and Belvidere Road.
Lubeno was a passenger on a 1996 Harley-Davidson motorcycle driven by her husband, James.
He was also hospitalized at St. Therese.
Luff said Porcayo drove onto Belvidere Road from a restaurant into the path of the eastbound motorcycle, which struck the rear wheel and quarter panel of the truck.
The Lubenos were thrown from the motorcycle.
After the collision, Porcayo stopped his truck, left the keys in the vehicle and fled on foot, Luff said.
He was detained by an Ameritech crew working in the area who turned him over to police.
On  Saturday, Porcayo was released on bond posted in the police station after he was charged with driving without a license, driving without insurance and failing to give aid or assistance at the accident scene, Luff said.
Additional charges are pending against Porcayo because of Lubeno’s death, Luff said, but police now are not sure of his identity.
Additional information has been developed indicating that he may be Javier Porcayo or a man whose last name is Porcayo-Garcia, Luff said.
Initial efforts to locate the man were unsuccessful Tuesday, Luff said.
The police chief is seeking witnesses to the crash and information from the public on the man’s identity.

She was employed for the past 18 years at Quill Corp. in Lincolnshire.
